In this pivotal position, you will play a key role in ensuring continuity of care and maintaining the highest standards across each patient’s individual care journey. You will be responsible for coordinating complex admissions and leading comprehensive discharge planning for patients admitted to our statewide Spinal Injuries Unit, supporting seamless transitions and exceptional clinical outcomes. The purpose of the Clinical Care Coordinator – Clinical Nurse Specialist Grade 2, Spinal Injuries Unit is to maintain continuity of care and to facilitate a high standard of care throughout the patient individual care journey. The position will coordinate complex admissions and discharge planning for patients admitted to the Prince of Wales statewide service Spinal Injuries Unit. As a Clinical Care Coordinator – Clinical Nurse Specialist Grade 2 in SESLHD you will provide individualised, case‑managed complex patient care and provide leadership for the Spinal Injuries Unit at the Prince of Wales hospital to maximise the health outcomes for the patient and their families. You will exercise extended autonomy of decision‑making and professional knowledge and judgement in providing complex care requiring advanced clinical skills. You will be recognised as a healthcare professional employing your clinical skills and critical thinking in delivering high quality patient care. You will lead the development of the nursing specialty clinical practice and service delivery in the Spinal Injuries Unit, be an integral part of the interdisciplinary team in the provision of person‑centred care, and lead, support and supervise other nurses and care roles. Key Functions Complex admission and discharge planning of patients within the statewide spinal injuries’ unit service Collaborate with the Spinal injuries multidisciplinary team Attend relevant admission intake and discharge planning meetings Collaborate with the patient flow team by providing updates on the Spinal waitlist, communicating with external facilities ensuring smooth patient flow is maintained within the service Engage with patients, families or carers by providing specific spinal support and education Where You'll be Working Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Randwick Local Council is renowned for our world‑class beaches and supported by cafes and restaurants … Selection Criteria Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse. Relevant post‑registration qualifications and at least 3 years’ experience working in Spinal Injury or Spinal Rehabilitation, or such other qualifications or equivalent experience deemed appropriate. Demonstrated ability to manage complex clinical care and service delivery issues and problem‑solve through the development and use of clinical management plans. Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations. Demonstrated skills in the development and facilitation of patient and carer education; and ability to mentor and coach nursing staff to enhance professional development. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others. Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
