Collaborate with clients, families, and multidisciplinary teams to create person-centred care plans and ensure holistic support. Communicate effectively with stakeholders. Deliver individual and group therapy sessions to enhance daily living skills, cognitive abilities, mobility, and self-care. Provide education and support to clients, caregivers, and support workers. Maintain accurate and up-to-date client records and documentation. Stay informed about the latest research, best practices, and regulatory changes. Participate in team meetings, case conferences, and professional development activities, and contribute to a positive workplace culture.
