About the Program Moving On provides culturally appropriate support and brokerage assistance to young people aged 16 – 25 years, who have transitioned to independence from OOHC in the Northern Territory. About the Opportunity As the Case Manager you will deliver strengths based, and culturally safe information, assessment, referral, advocacy, support, group work and brokerage assistance to young people aged 16 – 25 years, who have left the care of Territory Families CEO. You will work collaboratively with various stakeholders to ensure comprehensive support and effective outcomes, empowering them to achieve independence and reach their full potential. Responsibilities Deliver strengths‑based, culturally safe support and brokerage assistance to young people; provide information, assessment, referral, advocacy, support and group work; manage crisis situations; collaborate with stakeholders; ensure safe, respectful, and culturally competent care. Qualifications Relevant qualifications in Human Services/Social Sciences combined with direct experience working alongside high‑risk youth & families in a community‑based environment. Demonstrated understanding of trauma‑informed, strengths‑based practice and case‑management approaches, combined with strong youth engagement skills. Demonstrated ability to manage crisis situations and make good decisions under pressure. Ability to work respectfully and collaboratively with Aboriginal and Torres Strait Islander peoples, their families, communities, staff, and Aboriginal controlled organisations.
