Nurse Navigator (Homeless Health Outreach Team) Gold Coast Hospital & Health Service – Southport QLD An opportunity has become available for a Nurse Navigator (Homeless Health Outreach Team) to join Gold Coast Health (GCH) where the focus of our 13 000-strong team is on providing world‑class health care to people living in the Gold Coast, and northern New South Wales region of Australia. What you will do Delivery of positive consumer‑centred health care for individuals. Navigate the health system and work across service teams and systems to coordinate the care needs of the individual. Establish effective discharge pathways for consumers. Apply advanced clinical and professional practice capabilities within mental health services. About you A Bachelor of Nursing or equivalent. Registration with the Nursing & Midwifery Board of Australia. Credentialed through the College of Mental Health Nurses (or be prepared to work towards a credential in Mental Health Nursing). Master's or higher qualification within a relevant area is highly desirable. Driver Licence. Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against – Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B. Tuberculosis screening, to identify if the tuberculosis vaccination may be required. Declaration of serious discipline history must be disclosed. About Us This position is located in the Emergency Department and co‑located within the Homeless Health Outreach Team (HHOT), Mental Health and Specialist Services (MHSS). The Nurse Navigator role will work as the interface between the HHOT and Emergency Department (ED), specifically, to identify and coordinate clinical practice for homeless persons who are experiencing mental illness and present to the ED. The role provides high quality, evidence based nursing care, clinical leadership and education to identify and support homeless consumers and to improve the health and well‑being of homeless consumers that experience mental health illness. This role will provide for continuum of care, deliver mental health recovery support program models of care and both identify and support high frequency ED presenters and consumers with mental health related comorbidities.
