The Role: Our Clinical Coordinator provides clinical leadership and governance of our PEACH (Palliative Care Home Support Packages) program as part of our Home and Community Support team. The Clinical Coordinator works as a senior nursing clinician to provide a variety of nursing duties, including coordination, medication management, assessment, and case management and supervision of staff to ensure appropriate care for our clients with life-limiting illness.

Key Accountabilities:

  • Provide clinical leadership and supervision of care teams including Registered Nurses, Enrolled Nurses, and Personal Care Workers.
  • Promote a culture of service delivery that aims to improve the lifestyle experiences of client.
  • Assist with the management of the Master roster ensuring the appropriate model of care is provided at all times.
  • Promote client’s rights and responsibilities and liaise with external stakeholders ensuring prompt resolution of any enquiries, feedback, clinical matters requiring escalation or complaints.
  • Supervise and support staff reporting to this position, including competency assessment and identification of training need.
  • Assist with the management of clinical stocks ensuring availability and compliance within budget and program requirements

What do we need from you?

  • AHPRA registration as a Registered Nurse
  • Minimum 5 years’ experience as a Registered Nurse
  • Minimum 1 years’ experience in community nursing and/or palliative care (essential)
  • Well-developed problem-solving skills particularly when dealing with complex care scenario’s
  • Excellent communication skills both written and verbal
  • Excellent organisational and time management skills
  • Working with Children Check (or willingness to obtain)
  • Own vehicle/licence
  • Evidence of two COVID-19 vaccinations
  • Be available to work evening shifts.
