Medela Health is a growing NDIS provider specialising in complex and high-needs supports, including Autism Level 3, Schizophrenia and complex mental health, Cerebral Palsy, and Motor Neurone Disease (MND) . We work closely with participants, families, Support Coordinators, Behaviour Support Practitioners and Allied Health to deliver safe, consistent and highly individualised supports. We are currently seeking an experienced Service Coordinator to join our team. About the Role Reporting to the General Manager , you will be the key link between participants and their families, support workers, clinicians and the organisation. You will manage a dedicated caseload, ensuring that the supports promised are the supports delivered, and that quality, safety and continuity of care remain central at all times. This role combines office-based coordination with field-based engagement and quality oversight. Key Responsibilities Act as the primary liaison between participants, families, support workers, clinicians and management Manage a caseload, ensuring service agreements, rosters and support plans are implemented effectively Coordinate supports for participants with complex physical, psychosocial and behavioural needs Conduct home visits to monitor quality of care and participant wellbeing Support the implementation and monitoring of Behaviour Support Plans and restrictive practice requirements in line with NDIS Commission obligations Complete accurate reporting, documentation and compliance records Work closely with rostering and leadership to ensure continuity and stability of supports About You You are organised, resilient and values-driven, with a strong understanding of the NDIS and experience supporting people with complex needs. You are confident working with risk, behaviour support, mental health, and high physical support requirements. nov12 You will bring Minimum 4 years’ experience working within the NDIS (mandatory) Experience supporting people with Autism Level 3, Schizophrenia, complex mental plano, Cerebral Palsy and/or MND Strong understanding of Behaviour Support, Restrictive Practices and NDIS Quality & Safeguards requirements Excellent written and verbal communication skills Ability to manage competing priorities and work under pressure A relevant qualification is preferred (e.g. Social Work, Psychology, Nursing, Allied Health or Community Services) Mandatory Requirements – Why Join Medela Health Specialist provider in complex disability and mental health Supportive(interactively valued) leadership Strong clinical collaboration with BSPs, OTs and allied health Opportunities for professional growth and development Meaningful work making a real difference in people’s lives #J-18808-Ljbffr
