About The Role Provide a comprehensive social work service to patients, clients and families across the Heart–Lung Stream, including Cardiology 1 & 2, Cardiothoracic Surgery, Thoracic Medicine, Thoracic and Cardiac Rehabilitation, and Heart and Lung Transplant (inpatient and outpatient), with flexibility to work according to clinical demand and priority. Work closely with multidisciplinary teams within St Vincent’s Hospital and external agencies, providing consultation, liaison and advocacy while contributing to high‑quality, patient‑centred care. Deliver clinical assessments, counselling and complex discharge planning, supporting patients, families and carers, and providing clinical cover for colleagues as required. Assist with education for patients and staff, and contribute to quality improvement activities and research. Ensure ongoing professional growth by attending departmental professional development activities and clinical supervision. Participate in the 7‑day, after‑hours on‑call social work crisis roster. About You Hold a degree in Social Work and be eligible for AASW membership, with demonstrated clinical experience or understanding of the role of social work in a tertiary hospital or health‑related setting. Clinical skills and knowledge in psychosocial assessment, counselling, therapeutic interventions, case management, advocacy and discharge planning. Understanding of and/or practical experience in crisis intervention, short‑term solution‑focused approaches and grief and loss support, with the ability to work effectively within multidisciplinary teams in high‑pressure environments. Excellent time management and prioritisation abilities, proficient computer skills, and strong written and verbal communication skills, with the capacity to manage competing demands and liaise effectively across services. Capacity to contribute to research initiatives, quality improvement programmes and educational activities. Willingness and ability to participate in extended hours and on‑call service as required. About The Hospital In this role you will be based at St Vincent’s Hospital, Darlinghurst, a busy inner‑city hospital in a beautiful location. Closeness to universities & affiliated research institutes, many of which are located on‑campus. Centers of excellence include heart & lung transplantation, bone marrow transplantation, cardiology, cancer care, AIDS/HIV, respiratory medicine, mental health, homeless health, drug & alcohol services, and correctional health.
