Advanced Practice Occupational Therapist

To this end, NHS Scotland welcomes applications from all sections of society. We are looking for an experienced Occupational Therapist to lead the East Ross Discharge to Assess team. The post holder will provide assessment and specialist interventions to adults in hospital, or recently discharged from hospital, in order to facilitate an early and supported transition from hospital to home. The team will work to improve the patient journey, from the initial point of a hospital stay, preventing any delays through early and effective planning and ongoing assessment at home. This will involve working with patients in their own home and providing rehabilitation plans for support staff to follow. Multidisciplinary and multi-agency working is an essential component of the post and, on occasion, there may be a request for you to work out with normal office hours, therefore a degree of flexibility is required. Applicants should have a good understanding of the Discharge to Assess philosophy and be experienced in team working, demonstrating effective communication skills. Experience of supervising other staff and a commitment to continued professional development are also essential. A driving licence and use for own car for work purposes is essential. You must be registered as an Occupational Therapist with the Health and Care Professions Council (HCPC).
