About the Role We are seeking a passionate and experienced Dietitian to join our team as a sub-contractor. In this role, you will primarily support clients under the National Disability Insurance Scheme (NDIS), as well as older adults receiving services through the Support at Home program and residential aged-care facilities. At VK Dietetics, around 80% of our caseload consists of NDIS participants, with the remaining 20% supporting aged care clients. In this role you will: Provide high-quality, evidence-based nutrition care across the disability and aged-care sectors, aligning with our person-centred and compassionate approach. Conduct individualised nutrition assessments and interventions for a wide range of clinical conditions. Conduct regular follow-up reviews to monitor the nutritional status of clients and residents and adjust nutrition plans as required. Undertake education sessions and menu reviews in residential aged care homes. Prepare NDIS-aligned reports, personalised meal plans, and tailored client resources. Maintain comprehensive and accurate records and documentation in line with professional standards. Work collaboratively with clients, their families, caregivers, support coordinators, allied-health professionals, and other services to support person-centred and high-quality nutritional care. Be comfortable working across both home settings (for NDIS and Support at Home clients), residential aged-care facilities and telehealth. What we are looking for: At least 6 months of experience working with NDIS participants (desirable), or A minimum of 1.5 years of experience practising as a dietitian in a hospital, community or private practice setting. Strong communication and interpersonal skills. An empathetic approach with excellent rapport-building abilities. Strong organisational and time-management skills. Flexibility and adaptability, with a willingness to travel across different regions in Melbourne and work in various settings. Ability to work independently and collaboratively within a multidisciplinary team. Willingness to take on an already established and diverse caseload.
