About the Role Our new role of Restorative Care Partner, reporting to the Manager – Complex and Clinical Care, is part of our Clinical Care team and plays a key role in shaping our Restorative Care Pathway under the new Support at Home program. Working closely with older Australians, you’ll deliver evidence-based, person-centred care that supports independence and wellbeing. This collaborative role offers the opportunity to contribute to a passionate team, apply your allied health expertise, and make a meaningful impact on the lives of our Members so they can continue living well in the homes and communities they love. The core responsibilities of the role include: Design and Develop Restorative Care Services: Lead and contribute to the development, rollout, and continuous improvement of the Restorative Care Pathway under Support at Home, collaborating with internal teams and sector partners to co-design innovative service models. Deliver Evidence-Based Clinical Support: Conduct functional and clinical assessments, develop personalised goal and exit plans, and coordinate therapeutic interventions through our partner network to support Member wellness and reablement. Empower Members and Their Support Networks: Educate and support Members, families, and Helpers with practical strategies to enhance independence and quality of life at home. Maintain Clinical and Program Knowledge: Stay informed about the Restorative Care Pathway program requirements, including support plan monitoring and funding optimisation, while maintaining accurate and timely clinical records. Collaborate and Drive Continuous Improvement: Work closely with multidisciplinary teams to ensure coordinated, person-centered care and actively contribute to service development, quality improvement, and professional learning initiatives. Do you have what it takes? Tertiary qualification in Physiotherapy or Occupational Therapy or another allied health discipline. Demonstrated knowledge, understanding and experience in these key areas: Current aged care reforms and the Support at Home program. Familiarity with the Aged Care Quality Standards. Restorative care and reablement approaches within the aged care context. Current registration with AHPRA (Australian Health Practitioner Regulation Agency) or similar. A police clearance certificate or willingness to apply. Current driver’s licence and willingness to undertake reasonable travel. Excellent interpersonal and communication skills, including the ability to build rapport with older people and their families. Ability to work both independently, and collaboratively/as a leader of a multidisciplinary team. What's in it for you?
