Position Summary: We are looking for a dedicated Community Worker to support clients in living safely and independently within their homes and communities. In this role, you will assess individual needs, develop tailored support plans and connect clients with the appropriate services and resources. You will provide practical and emotional support, build strong client relationships and work collaboratively with families, service providers and community organisations to ensure high-quality, coordinated care. This position is ideal for someone who is passionate about empowering people and making a meaningful impact in the community. Responsibilities: Conduct comprehensive client assessments to understand their individual circumstances, goals and support needs, and contribute to developing personalised care or support plans. Engage with clients through structured conversations and ongoing check-ins to identify challenges they may be facing—social, emotional, financial or health-related. Track and review client progress regularly, ensuring their support plans remain effective and updating documentation and reports in a timely and accurate manner. Connect clients with relevant community, government or specialist services, helping them navigate available resources and ensuring they receive the right level of assistance. Evaluate broader community needs and available programs, identifying services related to health, housing, training, employment and wellbeing that may benefit clients. Collaborate with community organisations, welfare groups and government services to strengthen support networks and raise awareness of available resources. Provide practical support to families, children, seniors and individuals with disabilities, contributing to a safe, supportive and inclusive environment. Support vulnerable or at-risk individuals, including young people, by helping them address barriers such as emotional stress, financial hardship or social isolation. Prepare clear, professional reports, case notes and funding submissions, supporting organisational compliance, program development and stakeholder communication. Requirements: Minimum qualification: Bachelor’s degree in Nursing Highly preferred: Master’s degree in Nursing or a related discipline Work experience: At least 4–5 years of relevant experience in community services, healthcare, or client support roles Strong assessment, case management and client engagement skills Excellent communication, problem-solving and interpersonal abilities Ability to work independently and collaboratively within a multidisciplinary team Commitment to person-centered care, cultural sensitivity and professional ethics #J-18808-Ljbffr
