Permanent Full Time, 80 hours per fortnight Located at the Mornington Centre On 1st January 2026, five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health; a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians. The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1st January 2026, all employees from the five health services will transfer to Bayside Health. The Team & Role Peninsula Health is seeking a permanent & full-time Grade 2 Social Worker to join our team at The Mornington Centre. In this role you will provide support to older adults who are receiving inpatient care in a GEM setting. You will be responsible for completing psychosocial assessments, providing emotional and adjustment support and provide targeted interventions with both individuals and their families. You will be required to have a sound knowledge of the psychosocial issues that older people in our local community experience & will have experience in navigating relevant service systems. You will have demonstrated experience in working collaboratively to achieve patient goals and develop patient-centred discharge plans. You will receive regular supervision and support from a dedicated on-site Senior Social Worker, and you will also provide supervision to Grade 1 Social Workers and students. Skills and Experience Required We anticipate you will have: You will need to hold a Bachelor Degree or Qualifying Masters in Social Work and two years of relevant post-qualification work experience. Postgraduate studies are highly regarded and may be eligible for a higher education allowance. As a multi-site organisation you may be required to work across sites.
