We are seeking a vibrant, committed and compassionate team member to join our Northwest Regional Office in Somerset in the role of Case Coordinator. Forming part of the Youth and Communities Team, the Case Coordinator works alongside our Vincentian Members, providing emergency relief assistance to those engaging with our services. Providing support, training and mentoring to members is key to this role, while assisting with appropriate referrals and resource sharing. The Case Coordinator also leads any complex case meetings, collaborating with members and staff to reach the best servicing decisions and outcomes for the companion. With a blend of face‑to‑face case coordination, member support, mentoring and administration‑based tasks; this role provides variety with direct connection to the community. This role is required five days a week, 9.30am till 2.30pm to suit the needs of our community. Experience dealing with cohorts of people that are facing extreme barriers or disadvantage via a case management, client‑centered or coordination model. Experience in the social or community services sector Demonstrated experience in documenting objective case management notes Ability to work alongside our Members and Volunteers, providing support and training when required Knowledge and competency of de‑escalation techniques, along with varied communication styles and tools Competent with Microsoft applications Ability to manage competing deadlines and priorities, adapting as required A can do attitude and commitment to the values of St Vincent de Paul Society Knowledge of and willingness to further learn about social justice issues Strong commitment to confidentiality Ability to build, foster and manage relationships with key stakeholders and other like minded organisations Willingness to assist with event management or attend representing The Society Motivation to be part of the greater Society Team and collaborate appropriately The successful applicant must hold a current compliant National Police Check, along with a valid Working with Vulnerable People Card or the ability to obtain.
