Main duties of the job The post holder will need to be an experienced clinician who has ideally had experience working in primary care, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. With appropriate clinical assistance from the clinical hub and duty doctor, they will demonstrate safe, clinical decision‑making and expert care for patients within the general practice. They will work as part of a multi‑disciplinary team in a patient‑facing role, take responsibility for areas of chronic disease management within the practice, undertake clinical assessments, diagnostics and treatment either in the practice or at a patient’s residence, and provide diagnostics and assistance using broad clinical guidelines such as NICE. Primary support to general practice staff includes accurate triage, prioritising patients, including in emergency situations. The post holder will provide clinical leadership, pursue quality improvement and manage aspects of the quality and outcomes framework. They will ensure integration with community and local service providers to utilise skill mix, improve patient outcomes, ensure better access, and help manage workload. Person Specification – Experience Essential Clinical experience, recent experience in ambulance services, emergency care, or pre‑hospital care. Strong assessment and clinical decision‑making skills. Primary Care/Community knowledge. Understanding of primary care pathways and community services. Ability to manage minor illnesses and injuries safely. Competence in history taking, examination, and assessment. Ability to make autonomous decisions in a primary care setting. Basic life support and emergency care skills. Excellent verbal and written communication with patients and colleagues. Competence in electronic patient records (e.g., SystmOne, EMIS). Accurate clinical documentation and record‑keeping. Ability to work independently and as part of a multidisciplinary team. Desirable Advanced clinical training. Postgraduate or advanced practice courses, e.g., MSc Advanced Clinical Practice. Independent prescribing (V300) qualification. Primary Care experience. Previous work in GP surgeries, urgent care centres, or community teams. Experience in minor illness/injury clinics, chronic disease management, or home visits. ECG interpretation, phlebotomy, wound care, or vaccinations. Mentorship or practice educator experience. Quality improvement or audit projects. Research or evidence‑based practice. Participation in clinical audits, service evaluation, or research. Additional courses such as Advanced Life Support, PHTLS/ITLS, Safeguarding, Mental Health First Aid. Qualifications – Essential Registered with the HCPC with an approved paramedic science degree or diploma. Ongoing continuing professional development (CPD). Adherence to the HCPC standards of conduct, performance and ethics. Qualifications – Desirable Postgraduate level (Level 7) postgraduate diploma or MSc in Advanced Clinical Practice. Minor illness / Minor injury courses.
