We are looking for an experienced Support Coordinator to join our growing team in Perth on a permanent full-time position. We are seeking motivated people who have a genuine approach and proven ability to support people with disability. This role involves assisting people to implement and coordinate their NDIS plans to build the way to a better life and optimise outcomes to achieve their NDIS goals. Key Responsibilities as a Support Coordinator Assist NDIS participants in understanding and implementing their NDIS Plans Connect participants with required services and supports such as NDIS providers, community resources, government services, and informal supports like family and friends. Complete all necessary administration to the required standard, ensuring full compliance. Arrange and monitor services by locating suitable providers, negotiating service agreements, coordinating schedules, and ensuring supports are delivered effectively. Assist participants to build capacity to manage their own plans where possible, develop skills for making decisions, and prepare for future NDIS reviews. Manage challenges and assist to overcome barriers to accessing services and provide strategies to avoid or respond to crises. Monitor and report on the progress of participants plans and prepare and complete reports for NDIS meetings or reassessments. Promote ISA to the market and generate interest in support coordination under the NDIS. This may involve delivering presentations to various organizations such as Local Area Coordinators, Disability Service Providers, and support groups, including participants and nominees. About You Strong working knowledge of the NDIS to help people understand their NDIS plans and navigate the NDIS environment. Building positive working relationships with clinical and other government, community and local agencies to support people in a holistic way. Your empathy and care for people. Your ability to effectively present information and respond to questions from groups, managers, clients, and customers. Your excellent verbal and written communication skills. Your strong acumen in engaging potential clients and securing leads from inquiries. Your ability to work independently and as part of a team. You are a people person and are able to engage and build relationships with a variety of people. You have sound self-management skills to prioritise, account for and manage time effectively. You want to make a difference in people's life. Requirements Current driver's license and willingness to travel to clients where required. Minimum 1 year of experience working as an NDIS Support Coordinator. Allied Health qualifications or a minimum Certificate IV in Allied Health, Disability, Aged Care, or similar. Experience or knowledge in complex family health issues would be beneficial. Successful applicants will be required to undergo an NDIS Worker Screening Check and provide a current Working with Children Check.
