About the role We are looking for a skilled, enthusiastic and dedicated Commonwealth Home Support / DVA / VHC Care Case Manager to join our Home Care Team in Launceston. The role requires regular travel to Shearwater. Responsibilities Provide day‑to‑day management of a client case load, care planning and delivery of consumer‑directed services. Make a real difference to our consumers’ lives. Enable consumers to remain in their own homes. What you bring to the role Relevant experience in case management, community services, aged care or equivalent (Essential). Knowledge of CHSP and VHC funding (Essential). Experience in service delivery to clients living in the community. High level planning and organisational skills. Well‑developed communication skills. A desire to create a positive experience for our consumers. A current unrestricted driver’s licence (Essential). Evidence of NDIS Endorsed Working With Vulnerable People Check (essential) and Covid‑19 vaccination (if applicable).
