Support older adults to live independently in the community. Work across diverse WSLHD sites in a multidisciplinary team. Deliver impactful care for chronic and complex aged conditions. Support older clients with chronic needs to regain mobility & independence through quality physiotherapy within a collaborative CAC team. Employment Type: Permanent Part Time Position Classification: Physiotherapist Level 1/2 Remuneration: $38.51 - $54.94 per hour Hours Per Week: 24 Location: Auburn Community Health Centre Where you'll be working Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing Chronic and Complex (CAC) Allied Health services focus on restoring, improving, or maintaining older people’s independent functioning and wellbeing. The focus of these services is assisting older people to regain or maintain physical, functional, and cognitive abilities which support them to either maintain or recover a level of independence, allowing them to remain living in the community. CAC Physiotherapists are responsible for the assessment, intervention/treatment, and education of quality physiotherapy intervention for clients with complex, aged and chronic issues residing within the Western Sydney Local Health District to promote restoration and improvement in physical wellbeing and mobility. CAC Physiotherapists are expected to work from different locations across WSLHD as reasonably required. Essential Criteria Qualification: Tertiary qualifications in Physiotherapy. Registration: Current general Physiotherapy registration under AHPRA. Selection Criteria Demonstrated experience and skills in physiotherapy assessment, planning and evaluating interventions for clients with chronic and complex health conditions. Demonstrated understanding of the health and associated care needs of persons with complex, chronic and/or aged conditions residing in the community. Demonstrated ability in the use of relevant evidence based, best practice guidelines to guide decision making for best client outcomes. High-level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases. Ability to work independently and as part of multidisciplinary team. Current drivers licence valid within Australia.
