Join the caring and professional Country Health Connect team as a Palliative Care Clinical Nurse, delivering high-quality, person-centred care across the Barossa Hills Fleurieu region. The Casual Pool has opportunities across Barossa/Gawler, Adelaide Hills, and Southern Fleurieu/Kangaroo Island , giving you the flexibility to work in communities you enjoy while supporting clients to remain at home for as long as possible. In this role, you will assess individual client needs, plan and coordinate care, and provide clinical guidance to less experienced staff. Assess, plan, implement and evaluate care to achieve the best possible outcomes for clients and families. Provide leadership, support and guidance to Enrolled Nurses, students and less experienced staff.
