Seslhd - Staff Specialist - Community Connection Physician

The Prince of Wales Hospital Community Assessment Unit has 0.3FTE available for a Staff Specialist. This position is temporary with the possibility of extension from 20th January 2025 through to 2nd November 2025.
The Community Connection Physician will provide care in the Community Assessment Unit (CAU) and to patients being managed in the community by the health service.
The Specialist will be offered an affiliation with an appropriate internal medicine subspecialty within Prince of Wales Hospital, aligning with existing community health programs. The Community Connection Physician will have demonstrated interest in general medical as they will provide expert medicine assessment and management of ALL patients within the CAU, advice and clinical support to the Community Management Centre (CMC) and whilst rostered for the On Call service for the CAR. They will also fulfil other subspecialty duties required by their affiliated and respective subspecialty Head of Department (HoD). The Community Connection Physician will maintain professional reporting lines to their affiliated and respective HoD.

SESLHD covers urban and suburban communities and manages eight public hospitals and one public nursing home:
Calvary Healthcare Sydney (third schedule with Little Company of Mary Health Care) Prince of Wales Hospital Royal Hospital for Women St George Hospital Sutherland Hospital Sydney/Sydney Eye Hospital War Memorial Hospital (third schedule with Uniting Care) Gower Wilson Memorial Garrawarra Centre SESLHD also operates 28 Child and Family Health Centres, 12 Community Health Centres and nine Oral Health Clinics; providing prevention, early intervention and community-based treatment, palliative care and rehabilitation services.
Drug and Alcohol, Mental Health, Breast screening, HIV/AIDS, Public Health, Women's Health, Youth Health, Sexual Health, Imaging and Pathology services are provided at a number of facilities across SESLHD.
The primary location of this position may be varied as agreed with the Director of Clinical Services and as approved by MDAAC to other facilities in SESLHD.

Registration or eligible for registration with Medical Board of Australia (AHPRA). Candidate to provide AHPRA Registration Number on application. Fellowship of The Royal Australasian College of Physicians and/or equivalent specialist recognition as provided for by the Health Insurance Act 1973 and the relevant Award/Determination, with demonstrated interest in general medicine and demonstrated subspecialty training in specialties with existing links to community health programs. Evidence of continuing medical education and self-monitoring/audit practices consistent with the scope of practice requirements of the position. Proven capacity and/or experience in the provision of effective high-quality service in the specialty, demonstrated through successful clinical practice in an environment appropriate to the requirements of the position including evidence of participation in quality improvement activities relevant to the specialty. Demonstrated commitment to teaching with experience in teaching both undergraduate and postgraduate students and other staff of the hospital. Demonstrate the skills and attitudes appropriate as a senior Medical/Dental Practitioner including modelling a high standard of professional behaviour to junior medical and dental staff. Demonstrated ability to work effectively and harmoniously with medical and other health services colleagues as part of a multi-disciplinary health care team and demonstrated ability to communicate effectively with patients, colleagues, and staff and to work as an effective member of the clinical team across multiple sites. Demonstrated commitment to provision of a high standard of clinical care in the public sector setting. Demonstrated experience in and commitment to research relevant to the role and needs of the Department. This is a Category A position. All Category A applicants must read and understand NSW Health Policy PD2023_022.
