Alfred Health Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory to inpatient and home and community-based services. On 1 January Alfred Health will come together with Kooweerup Regional Health Service, Peninsula Health, Bass Coast Health and Gippsland Southern Health Service to create Bayside Health, a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians. The Department The Commonwealth Department of Health and Aged Care has established a new Single Assessment System (SAS) to improve access to aged care services. As of January This system has replaced the Aged Care Assessment Service (ACAS) and Regional Assessment Service (RAS). Alfred Health Aged Care SAS provides comprehensive and home support assessments via the Victorian Dept of Health agreement. Comprehensive assessments are usually completed face to face in the hospital or community setting, to determine a client's eligibility for aged care types under the Act. Home support assessments are completed in a community setting for clients identified as having entry level needs that can be accommodated by Commonwealth Home Support Services. The geographical catchment for community assessments incorporates the 'Southern Metro Aged Care Planning Region'. The Role This role will require you to travel to complete face‑to‑face assessments as well as telehealth and phone assessments as required. You will need to be adaptable to assessment demand and may be required to complete home support assessments. You will be involved in training for the new Aged Care Act and Support at Home Program which commenced 1st November. Experience and Qualifications Required Suitable qualified health professionals: Nurse, Occupational Therapist, Physiotherapist, Social Worker. Must have advanced assessment skills, community and aged care experience. Degree in a relevant Allied Health discipline and AHPRA registered or eligible for professional membership if a self‑regulated Allied Health profession. Degree in Bachelor of Nursing or equivalent and AHPRA registered. Highly desirable Recent SAS, ACAS or RAS experience and current compliance with all My Aged Care (MAC) learning modules.
