DFV Practitioner - Mobile Outreach (4 - 5 days/wk) Chisolm Inc is seeking a compassionate and experienced DFV Practitioner to join our team and broaden our mobile outreach program. The position is offered on either a 4 day per week or 5 day per week permanent basis (dependent on the applicant). Who we are We support women and children who are experiencing domestic and family violence with practical and emotional assistance to attain self-determination and independence. We aim to disrupt the cycle of violence both now and for future generations. Our vision is a future free of violence and abuse for every woman and every child within our community. We work to reduce the impact of Domestic and Family Violence on women, children, young people and their families by assisting them to access safe, supported accommodation and independent living. We achieve this through providing high quality services, which are culturally inclusive, non-judgmental, respectful, client focused and provide an opportunity for women, children and young people to make informed choices regarding their future. We strive to work from a framework which incorporates community development, feminist and social justice principles. Who you are The successful candidate will be responsible for providing case management services to individuals and families affected by domestic and family violence - meeting with them primarily in the community but will also support refuge clients as needed. They will work closely with the Manager to expand the mobile support program and will require strong networking and client skills. Key Responsibilities: Provide comprehensive case management services to clients experiencing domestic and family violence. Conduct risk assessments, personalized safety planning, referrals, advocacy, and coordination of services to support clients in achieving their goals through a strength based approach. Group work with clients on topics such as DFV healing and recovery and parenting skills. Advocate on behalf of clients with external agencies, including police, legal services, housing, child safety and healthcare providers. Coordinate access to relevant services and resources, including emotional support, financial assistance, housing, and legal support - attending with them at these services including police stations, court etc as needed. Maintain accurate and confidential client records, documenting case notes, assessments, and progress reports. Work collaboratively with a multidisciplinary team to ensure clients receive holistic support. Stay informed about current best practices, policies, and legislation related to domestic and family violence. Qualifications and Skills: Bachelor’s degree in Social Work, Human Services or a related field. Minimum of one year of experience in domestic and family violence, crisis intervention, or a related area. Strong understanding of the dynamics of domestic and family violence and trauma-informed care principles and understanding that DFV is a gendered issue. Excellent communication, advocacy, and negotiation skills. Ability to work independently and as part of a team in a high-stress environment. Proficiency in maintaining accurate case management records and using relevant software. Current driver’s licence, Blue Card, First Aid and CPR.
