Care Partner

What You’ll Do: Manage and coordinate individual care plans for aged and disabled participants. Assess participant needs and develop tailored support strategies. Collaborate with participants, their families, and service providers to ensure optimal care delivery. Monitor participant progress and adjust care plans as needed. Oversee participant and service budget monitoring. Promote independence and community engagement for our participants. Work closely with a supportive and professional team to deliver exceptional service. What We’re Looking For: Minimum of 2 years’ experience within the past 10 years working as part of a team responsible for community-based aged care and disability services. Demonstrated advanced working knowledge and experience of the Home and Community Care Programs. Experience in working with participants and carers from culturally diverse backgrounds and those with dementia, frail aged, disability and mental health. Proven ability in participant and service budget monitoring. Compassionate, respectful, and excellent communication skills. Demonstrated experience identifying and working effectively with complex consumer care needs including social economic issues, mental health, substance and/or elderly abuse.
