We are Just the Place to grow your career and be part of a valued team delivering care where it\'s needed most Responsibilities If you are selected, you will: Work as a key member of a multidisciplinary health centre team to provide flexible, holistic, and culturally sensitive health care services to Aboriginal and/or Torres Strait Islander people and apply key strategies for face-to-face patient care. Focus on prevention and management of chronic disease for Aboriginal clients and support them on their journey throughout custody back into the community, particularly those with chronic or complex health conditions. Collaborate with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations and other relevant health and community groups to ensure seamless, timely and effective health care delivery. Location Silverwater Women’s Correctional Centre is a secure facility dedicated to housing female offenders. Located within the Silverwater Correctional Complex in Sydney’s western suburbs, it provides accommodation, support, and rehabilitation programs tailored specifically for women. The centre offers access to healthcare, education, and community services, playing a key role in the management and rehabilitation of female inmates in the metropolitan area.
