The role provides compassionate, high-quality nursing care to patients with life-limiting illnesses in their homes or community settings. The nurse works collaboratively with multidisciplinary teams to ensure holistic and patient-centered care. This is a part-time position focused on symptom management, emotional support, and improving the quality of life for patients and their families. Responsibilities Provide nursing care to patients with life-limiting illnesses in home or community settings. Focus on symptom management, emotional support, and quality of life for patients and families. Collaborate with multidisciplinary teams to ensure holistic, patient-centered care.
