Aged Care Manager

Just Better Care Central Coast is currently recruiting for an Aged Care Manager to join our caring and dynamic office team in Gosford, NSW. This is a permanent full-time role (38 hours per week, Monday–Friday). About the role: The Aged Care Manager leads the delivery, coordination and oversight of individualised aged care services. This role is responsible for care planning, service coordination, budget oversight, quality assurance, stakeholder communication, and ensuring safe, high-quality, person‑centred care aligned with Support at Home and the Aged Care Quality Standards. You will support participants and families, develop team capability, and ensure all services meet organisational, contractual and legislative requirements. Aged Care Home Care Management experience is essential, ideally with a minimum of 3–5 years in a comparable role. Key Responsibilities: Provide explanations regarding service agreements, budgets, rights and handbooks. Coordinate services with internal and external stakeholders. Monitor and review client needs and trigger reassessments. Coordinate service delivery and ensure staff are trained appropriately. Provide case management and financial oversight, recording all care management activity and oversight of the clinical care management billing. Conduct assessments and develop, monitor and review individualised care and health plans in conjunction with the Clinical Team. Develop and update care plans with participants and families in conjunction with the Clinical Team. Facilitate access to alternative services and community connections. Build and maintain stakeholder relationships. Manage compliance of associated providers and suppliers. Process invoices and monitor budgets. Provide leadership, mentoring and supervision to the team. Participate in client intake processes including Support At Home and Private Aged Care clients. Ensure compliance with Aged Care Quality Standards, Disability Standards and WHS. Participate in incident management, risk management and audits. Represent Just Better Care at external forums. Promote culturally appropriate, trauma-informed and person‑centred care. Assist with budgeting and care transitions. Assist with the transition to Support at Home (November 2025). Manage complaints, incidents and hazards. Perform other duties as required. Qualifications: Certificate IV in Aged Care. 3–5 years management experience in community aged care. Strong understanding of continuous improvement, Aged Care/Support At Home guidelines and Accreditation Standards. Experience coordinating staff and allied health practitioners. Ability to build strong community and health service networks. Strong administrative, written and verbal communication skills. Experience with electronic care management systems and coordinating service delivery. Current Working with Children Check and NDIS Worker Screening Check. Valid NSW Driver’s Licence.
