Specialist Dietitian - Community Diabetes The closing date is 27 January 2026 Liverpool Diabetes Partnership is a Community based service providing support to People Living with Diabetes in the Liverpool area. As a Band 6 Community based Specialist Diabetes Dietitian you will provide dietary education, support and individualised care plans to our patients. As a Community Service we deliver across a variety of community clinic, domiciliary visits and with the communities across Liverpool, providing virtual and face to face contact, group education for people with Type 2 and Type 1 diabetes as a registered DAFNE site. Supported by an Advanced Diabetes Dietitian and other MDT colleagues in the Service, working collaboratively with other Community Service to deliver care closer to home, and the NHS 10 year plan. The successful candidate will be involved with supervision of Student and Graduate Dietitians so strong leadership and communication skills are essential. You will be supported in your professional development and offered opportunities to develop skills in other areas. You will work closely with the Service Lead to identify any quality issues within the service and provide education to staff and colleagues in your Specialist area. We are a progressive team and with a pro‑active approach to service development and have created a supportive environment to encourage professional development. Annex 21 structured developed progression to Band 6 will be considered for current and newly qualified Band 5 dietitians without diabetes experience. Main duties of the job This is an exciting post where you would be involved in a Community based Multi Disciplinary Team (MDT) working in a forward thinking team of Diabetes Specialist Dietitians, Nurses and Consultants you will have the opportunity to develop your knowledge and skills in a variety of areas such as: Dietary education for patients with Type 1 and Type 2 diabetes utilising digital, written and face to face education. Input into MDT Diabetes clinics across Liverpool City supported by Diabetes Specialist Nurse and Consultants. Delivering education and support to Nursing and Care Homes across Liverpool. Harnessing new technologies and developments in Diabetes practice such as new injectable therapies, blood glucose data analysis in real time and insulin pumps. Involvement in audit, key performance indicators and service development. Engage with local communities to promote equity in the access to care and develop culturally relevant resources, education and peer support. Job responsibilities Ability to manage and prioritise own workload Experience of communicating and working effectively with service users, carers, other agencies and professionals Experience of establishing and maintaining good standards of professional practice to ensure the delivery of an effective, quality service Computer literacy and ability to use a variety of electronic systems Ability to work effectively as part of a team Ability to state the case for nutritional intervention as part of the care plan. Excellent presentation and communication skills Able to work as part of the MDT Good level of knowledge of a defined area of clinical practice and knowledge current best practice Evidence of continued professional development Evidence of developing knowledge within specialist areas of dietetics Person Specification Qualifications BSc in Nutrition and Dietetics or postgraduate Diploma. Current registration with HPC Membership of BDA or equivalent professional indemnity Introductory level Clinical Supervisory Skills Training Experience Broad clinical experience in at least 3 clinical areas, including medical and surgical patients Experience of multi-disciplinary working. Experience and evidence of continuing professional development. Experience of training student dietitians Experience of conducting audit Knowledge While able to work independently, ability to recognise own limitations and access advice and information appropriately. Skills Communication skills, both written and verbal, to effectively convey information Time management - able to prioritise work, respond appropriately to demands and keep to time in out patient setting I.T. Skills - power-point, keyboard and e-mail.
