We are looking for a Care Ambassador (Enrolled Nurse) to join our growing clinical and leadership team. Nurse Next Door is a premium home care service provider that enables people to live a fulfilling life at home for as long as possible, regardless of age or ability. We are dedicated to delivering flexible in-home nursing and caregiving to those in the Aged Care and Disability sector, ranging from 24/7 care to one-hour visits for wound care or diabetes management. We Make Lives Better every day for our clients and care team through our 4 core values : WOW Customer Experience Find a Better Way Passionate About Making a Difference Positions Available As our Care Ambassador you will work a 30 hour week with some on call duties. Our office is located in Osborne Park. As a Care Ambassador you are responsible for initiating Happier Ageing experiences for Clients and Caregivers as well as supporting the daily operations of the business. In every client care situation, you seek innovative solutions. You are able to with support from our Care Designers be involved in the care management from the first visit to the end of service. Your role will include but not be limited to: Caring Consults Client First Visits Continuity of Clinical Care End of Client Care People Leadership and management Complete records and documentation accurately and thoroughly We provide around the clock care so visits may be scheduled morning, afternoon or evening Some on-call required What you'll bring Always put clients first Are warm and caring Think of creative solutions and always find a better way Follow through with commitments and build great relationships Go above and beyond (even when no one is looking) Skills & Experience Community nursing experience would be an advantage Subacute and disability experience Ability to identify and report client care concerns to the Care Designer/Director of Clinical Services Diabetes and Wound Management Dementia experience would be an advantage Leadership and management experience is an advantage Flexibility and a can do attitude Flexible Working Hours Work with a supportive and dynamic team Mandatory Requirements Passionate about making a difference. A self-led leader with the ability to take responsibility for their own behaviour & success.
