Our model uses a comprehensive assessment battery and case conferencing process to inform targeted prescriptions, with the client journey culminating in ongoing exercise and service participation. Responsibilities Applying quality diagnostics and treatment skills, to the needs and wants of our clients Using best practice as well as evidence based tools to plan and deliver high quality individual and group exercise services tailored to clients assessed needs Contributing positively to multi-disciplinary care planning for our clients Engaging in case conferencing and problem-solving of complex care needs Proactively delivering evidence-based reablement pathways that ensure client physical, social and mental health benefits Demonstrated willingness to mentor students and to increase your personal skill set About you We are looking for people who will be the difference in the lives of our residents and clients. Must be an Accredited Exercise Physiologist with an appropriate tertiary level qualification and current full registration with Exercise and Sports Science Australia Demonstrated experience delivering health literacy to clients Well-developed technical skills to design, implement and evaluate safe and effective exercise interventions for older people Extensive knowledge and understanding of the effects of exercise interventions in managing chronic and complex clinical presentations Ability to teach, coach and motivate clients, using models of behaviour change, scientific evidence and clinical reasoning Have strong organisational and time management skills as well as being able to demonstrate initiative to work autonomously with minimal supervision Exceptional interpersonal skills including the ability to build and maintain strong internal and external stakeholder relationships Ability to maintain concise documentation Proficient computer and keyboard skills and the ability to use Google Suite (e.g. Docs, Sheets, Gmail etc.) and in-house databases. Commitment to professional self-development in the field of clinical exercise.
