Diabetes Nurse Educator (Darwin based – remote and urban services) Healthy Living NT seeks Diabetes Nurse Educators (Registered Nurses) to join its diabetes and cardiac education team, based in Darwin. There are multiple positions available which involve work in urban and remote locations. Positions can be part or full time. Relocation assistance from the PHN available for interstate applicants. Primary Objectives In coordination with other members of the Healthy Living NT education team and other members of the multidisciplinary team, an experienced diabetes nurse educator is expected to provide expert delivery of safe and effective evidence‑based clinical care and effective education to people with diabetes (Type 1, Type 2, GDM, IGT). Key operating areas include: Newly referred clients (group sessions and individual consults) General diabetes clinics (group and individual sessions) Remote services Act as a resource, organise and deliver educational programs to other health professionals including Aboriginal Health Practitioners, remote area nurses, practice nurses and GPs. Oversee the identification and development of diabetes educational resources, particularly for Aboriginal and Torres Strait Islander people and their health professionals. All performance and conduct of duties shall be carried out in accordance with the principles of best practice, and in line with Healthy Living NT’s Guiding Values and objectives identified in Healthy Living NT Strategic Plan. Responsible to the Education Manager. Salary to the range of $100,000 - $120,000 (Not including super + remote allowance) with generous salary packing available. Salary is based on employment type (full‑time or part‑time). Specific Accountabilities Provide individual and group education and counselling sessions for people with diabetes and their families or support network, in an urban and/or remote environment. Liaise with referring Medical Officers primarily GPs and other health professionals including Aboriginal Medical Services, hospitals, and rural and remote clinic staff. Travel to remote and regional areas of the Northern Territory to provide diabetes education in coordination with other medical and allied health staff. Contribute to the production of two quarterly newsletters, one targeting people with diabetes and the other targeting health professionals, especially those in rural and remote communities. Liaise with relevant stakeholders to organise, develop, promote, implement and evaluate educational programs. Deliver accredited training and in‑services to health professionals including Aboriginal Health Practitioners, remote area nurses, practice nurses and GPs to increase their diabetes education knowledge, skills and service delivery. Identify, review and evaluate new diabetes‑related literature and products that may be of benefit to people with diabetes. Periodically review available diabetes educational resources for currency and appropriateness. Where existing resources are unsuitable or non‑existent design, develop and produce resources that are culturally appropriate for all identified client groups, particularly for Aboriginal and Torres Strait Islander people. Participate and assist the Diabetes Education team to identify, implement, evaluate and revise education programmes in accordance with best practice and evidence‑based care. Participate in health promotion activities as required. Assist the cardiac educator in the delivery of the cardiac rehabilitation programme when necessary. Gather data, report and provide other documentation for the programme for evaluation and reporting purposes as required. Participate in Quality Improvement activities and contribute to Accreditation as needed. Accountable for own nursing practice and the maintenance of self‑education in diabetes management and education, stages of behaviour change, adult education and cardiac rehabilitation. Practice in accordance with legislation and common law, within the Guidelines of the Nursing and Midwifery Board of Australia. Responsible for other duties as directed by the Education Manager / CEO. Additional Requirements Occasionally work non‑standard hours Undertake travel within the NT, including driving where necessary Some limited interstate travel It is a requirement of employment with Healthy Living NT that all health professionals obtain and maintain: a satisfactory Police Criminal Check a Working with Children check (Ochre card) a Northern Territory Driver’s License. Selection Criteria Essential Appropriate tertiary qualifications in diabetes education with preference for people who have Credentialed Diabetes Nurse Educator status or are working towards it. Registered Nurse who has current national registration from the Australian Health Practitioner Regulation Agency and NT driver’s licence (or eligible to obtain). Demonstrated clinical experience that will ensure the provision of a comprehensive diabetes education service. Demonstrated and well‑developed understanding of adult education learning principles, presentation skills, stages of behaviour change and group education. Demonstrated ability to participate in planning, implementing and evaluating education programmes, resource development and health promotion activities. Demonstrated ability to work independently, excellent interpersonal skills and the ability to effectively participate as a member of a small multidisciplinary team within a cross‑cultural environment. Demonstrated skills with computer applications and a commitment to using information technology for communication, data collection, evaluation and resource development. Highly desirable Extensive experience in diabetes management in the community. Possess high motivation, strong initiative and time management skills. Understanding of the principles of quality management and demonstrated commitment to the process of continuous review and quality improvement. Demonstrated analytical and report writing skills.
