Were expanding and are looking for a dedicated Nurse to join our Access Aged Care team in east Perths suburb of Lesmurdie. Supportive culture : We value your ideas and back your professional development. What Youll Do Work alongside RACF care teams as the on-site Access Aged Care nurse. Facilitate GP rounds (both in-person and telehealth) to ensure smooth timely care. Prepare organise and triage referrals to maximise efficiency and outcomes. Conduct bedside assessments and clinical reviews. Act as the communication bridge between RACFs GPs and families. Deliver excellent customer service with empathy and professionalism at every interaction Build strong relationships with residents families and facility partners. What Youll Bring Diploma of Nursing and AHPRA registration. Aged Care experience (desirable). Strong organisational problem-solving and communication skills. Confidence with IT and documenting resident needs. A drivers licence and reliable vehicle.
