About The Role And Where You Will Be Working The Team Lead Bran Nue Dae is responsible for leading Support Workers and delivering high-quality respite, in-home, and community support services across Support at Home, CHSP, and the Breakfast Club programs operating from Bran Nue Dae. The role combines team leadership with hands‑on Care Coordinator responsibilities, including client intake, assessment, care planning, and ongoing service monitoring. The Team Lead ensures a culturally safe, person‑centred service that enhances client wellbeing, promotes independence, and strengthens community connection. We’ll Also Be Asking You To Undertake Care Coordinator responsibilities such as client intake, assessment, service onboarding, and ongoing care planning. Promote a positive, supportive team culture that encourages high performance and multidisciplinary cooperation. Oversee compliance management for Aged Care, ensuring robust data gathering and accountability across all stakeholders. Support complaint management and resolution processes, promoting open communication, transparency, and empathy. Manage revenue streams for the centre, ensuring income optimisation and compliance with relevant contracts and funding guidelines. Proactively promote Bran Nue Dae services within the local and wider community, identifying opportunities for service utilisation and business growth. To Be Considered For This Role, You’ll Need Cert III in Aged Care Minimum of 2 years’ experience supporting high-care clients in a community services or health sector Professional development skills: experience in coaching and mentoring staff to build service capability Demonstrated customer service orientation and continuous improvement focus Working With Us At Southern Cross Care WA, we are committed to being an employer of choice.
