As the Aboriginal Health Practitioner, you will be central to the delivery of coordinated, culturally safe, and patient-centred care that bridges hospital, virtual, and community-based services. This role supports the delivery of virtual care to enhance equity, access, patient flow, and service connection across the region. Working Monday to Friday, 8:30am to 5:00pm, you will: Support Aboriginal people transitioning from acute care to community-based and out-of-hospital programs Work as part of a multidisciplinary team (MDT) to deliver coordinated care tailored to the needs of Aboriginal people Facilitate timely and culturally safe access to health and social care services through expert care coordination and case management Promote equity by delivering virtual care solutions that improve service access and support continuity of care. You Will Be Instrumental In Preventing unnecessary Emergency Department presentations Promoting safe, effective out-of-hospital care coordination and service linkage in the community for Aboriginal people Strengthening continuity of care across health and social care systems Enhancing service integration to improve outcomes for vulnerable Aboriginal clients This role forms a key part of the Western Virtual Connected Care in the Community for Aboriginal people – Planned Care for Better Health program.
