Position The Shire of Manjimup is looking for an individual who has a commitment to enhancing the lives of older people and their family members/carers to join our friendly team at Manjimup Home and Community Care. This role is responsible for: Assessing individuals needs and service requirements while promoting physical and emotional wellbeing. Developing support plans for individuals residing in the Respite House and Support at Home participants. Assisting with clinical aspects of Support at Home participants support when required. Ensuring Support Workers and EN’s under supervision are appropriately skilled and understand consumer needs. Administering Schedule 8 medication and providing specialised nursing care as per consumers clinical support plans in the Respite House and in-home care. Providing high-quality supervision of the Respite House, ensuring the respite service runs effectively and efficiently on a day-to-day basis. Working flexibly, including day, weekend, and overnight on-call shifts as required. Liaising with medical professionals, allied health professionals, colleagues and other personnel to support best outcomes for individuals. Assisting with clinical training of Respite House and Community workers. Providing high-quality clinical supervision of aged care packages. Leading, motivating, and supervising Aged Care Support Workers delivering in-home and respite-based care as necessary. Reviewing and developing clinical policies and procedures in consultation with the manager. Ordering of clinical stock and consumables. Other projects as directed by the Manager Home and Community Care. Selection Criteria To be successful in this role you will need: Current AHPRA Registration as a Registered Nurse. 3+ years nursing experience within the Aged & Disability care sector. Demonstrated advanced clinical experience and expertise working with participants in the community. Understanding of care assessment in the Aged Care sector. Knowledge and experience with supporting people living with dementia or a disability. Excellent computer literacy and communication skills both verbally and written. Current 'C' class driver's licence and a registered reliable vehicle with full comprehensive insurance is required. Knowledge of Aged & Disability legislation and standards. Knowledge of reporting requirements as prescribed in the contracts with the relevant funding bodies.
