Churches of Christ in Queensland – Hervey Bay QLD Churches of Christ Home Care Bundaberg & Hervey Bay Permanent Full Time – 38 hours a week About the role Churches of Christ are currently looking for a Clinical Care Partner within our dynamic Home Care between 2 sites of Hervey Bay and Bundaberg team, you'll be the vital link between our clients and the new Support at Home program—ensuring older Australians receive the right care, at the right time, in the way that best suits them. This is a hands‑on leadership role where you'll guide a team of passionate Home Care Workers to deliver high‑quality, person‑centered care that upholds dignity, independence, and quality of life. You'll work closely with clients, their families, and external providers to tailor support and foster strong, trusted relationships. As a Care Partner, you will: Assess Client Needs: Conduct initial and ongoing assessments to understand physical, emotional, and social needs, identifying risks and tailoring care plans with clients and families. Care Planning: Develop, review, and update person‑centered care plans aligned with client goals and Support at Home funding. Coordinate Services: Organise in‑home supports and liaise with external providers to ensure timely, quality care. Advocate and Educate: Support clients to understand their funding, choices, and rights, and advocate for access to appropriate services. Budget Oversight: Monitor individual budgets and guide clients on effective use of funds. Relationship Building: Build strong connections with clients, carers, and providers, supporting smooth transitions and ongoing care. Compliance and Safety: Maintain accurate documentation, uphold aged care standards, and proactively manage risks. Who we're looking for You're a natural people leader with a heart for community care and a talent for getting the best out of your team. You're confident navigating the aged care space and thrive in a fast‑paced, mobile work environment. The role will see you support both our Bundaberg and Hervey Bay Service, working on opposite days with another experienced Service Manager. Bachelor of Nursing Current registration with AHPRA as a Registered Nurse Currently holds a Medication competency Currently holds, or willing to obtain a First Aid and CPR qualification Currently holds, or willing to obtain a current Police Check Currently holds, or willing to obtain a National Personal Insolvency Check A current driver's license and a reliable, registered, and comprehensively insured vehicle suitable for work purposes are essential for this role.(Reimbursement of $0.96 per KM is provided for use of own vehicle) Hold valid Australian Work Rights – Unfortunately we are not currently able to sponsor Importantly, you will be someone who closely aligns with our organisational values of: Integrity, Compassion, Excellence and Courage.
