Social Worker/Key Clinician - Aged Psychiatry Salary : Social Worker Grade 2 (YC42-YC45) + Superannuation + Salary Packaging About the role: Western Health's Aged Psychiatry Assessment and Treatment Team (APATT) provides specialist mental health support to adults aged 65+ living in the community. As a Social Worker/Key Clinician, you will deliver psychosocial assessments, recovery-oriented interventions, discharge planning, carer support, and crisis management. Working both autonomously and as part of a multidisciplinary team, you will play a pivotal role in enhancing the wellbeing, safety and recovery outcomes of older adults experiencing mental health challenges. Key responsibilities: Deliver comprehensive biopsychosocial assessments and develop recovery-oriented treatment plans Provide brief, medium and long-term therapeutic interventions Work collaboratively with consumers, carers, families and community supports Complete clinical documentation in line with service requirements Offer specialist social work consultation, including family/carer support, housing and resource navigation Participate in multidisciplinary reviews and contribute to service quality initiatives About you: Degree in Social Work and eligibility for AASW membership Minimum 12 months post-graduate social work experience (mental health experience highly regarded) Skilled in psychosocial assessment and therapeutic social work interventions Understanding of psychiatric illness, ageing-related health issues, and recovery-oriented practice Excellent communication, documentation and stakeholder engagement skills Position Description About Western Health Western Health is one of the largest healthcare providers in Victoria, caring for a vibrant, diverse and rapidly growing community of one million people in Melbourne's western suburbs.
