Nurse Practitioner / Transitional Nurse Practitioner - Geriatric Outreach Service Position: Nurse Practitioner (NP) / Transitional NP. We are seeking a highly motivated Nurse Practitioner (NP) or Transitional NP to join our dynamic team supporting aged care patients at Prince of Wales Hospital and with the Geriatric Outreach Service (GOS). This is a rewarding opportunity to work collaboratively with multidisciplinary teams to deliver advanced, person-centred care to older adults—both within the hospital and through outreach to Residential Aged Care facilities. The role involves clinical assessment, treatment planning, and support for complex geriatric presentations, with a focus on reducing hospital admissions and supporting continuity of care. The Aged Care Nurse Practitioner (NP)/Transitional Nurse Practitioner works within the Prince of Wales Hospital (POWH) Aged Care patients and with the GOS. The service provides advanced clinical assessment, consultation, referral pathways and development of management plans including hospital avoidance strategies for older people in the community, including Residential Aged Care facilities (RACF), who are at high risk of emergency department presentation or readmission to hospital. This position is based at Prince of Wales hospital Geriatric department and includes both day and evening coverage during weekdays and weekends. Where you'll be working Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state-of-the-art Emergency Department and Intensive Care Unit.
