Aboriginal or Torres Strait Islander applicants must demonstrate Aboriginal or Torres Strait Islander status in addition to addressing the selection criteria. We’re looking for a dedicated Aboriginal Health Worker or Practitioner to play a vital role in strengthening the connection between Aboriginal clients and the health services that support them. In this dynamic role, you’ll work across both clinical and community settings, providing culturally safe care for Aboriginal clients aged 17 and over through wellness checks, case coordination, and holistic support. You’ll collaborate closely with a multidisciplinary network – including Allied Health, NDIS providers, NGOs, and acute care teams – while also thriving as an independent practitioner backed by a committed and supportive workforce. What You’ll Do Deliver safe, effective, and culturally informed care in line with the NSW Health Aboriginal Health Worker Guidelines and NSW Health Decision Making Framework. Provide primary health care in partnership with multidisciplinary teams, addressing the physical, emotional, cultural, social, and environmental needs of Aboriginal clients. Plan, deliver, and review Aboriginal health programs, including health promotion, education, and disease prevention initiatives that are culturally appropriate and community-led. Apply your knowledge of local Aboriginal communities to ensure care is holistic, culturally safe, and responsive. Maintain accurate documentation, data collection, and record-keeping to support continuity and quality of care. Provide mentoring, cultural support, and guidance to colleagues, helping build cultural awareness and support across the District. Requirements Be willing to work towards obtaining a Certificate IV Aboriginal Primary Health Care (Practice) within 2 years of enrolment in course and commencement of employment and work towards AHPRA standards to meet Registration as an Aboriginal Health Practitioner through supported education, training and study leave. Current NSW Drivers Licence and the ability to travel within the local area and throughout Hunter New England NSW LHD (as necessary).
