Occupational Therapist – Home Care & Community Services Join a purpose-driven organisation committed to supporting older people and individuals with disabilities to live safely, confidently, and independently at home. We’re seeking a dedicated Occupational Therapist to provide high-quality, person-centred therapeutic services across our home care and community programs in Tasmania. In this rewarding role, you’ll work as part of a multidisciplinary team to assess client needs, implement goal-focused interventions, and support meaningful engagement in daily living. Key Responsibilities Conduct holistic OT assessments addressing physical, psychological, functional, social, and environmental needs. Deliver evidence-based occupational therapy interventions to enhance independence in personal, domestic, and community activities of daily living. Provide recommendations for home modifications, equipment, assistive technology, and adaptive strategies. Support clients through education, tailored therapy plans, and ongoing follow-up. Work collaboratively within a multidisciplinary team to maximise clients’ skills and promote safe, independent living. Liaise with families and carers to provide guidance on effective support strategies. Maintain accurate documentation, client statistics, and reporting in line with organisational and professional standards. Contribute to continuous improvement, service development, and quality initiatives. Participate in supervision of students and support staff where required. Promote safety and wellbeing by adhering to OH&S policies and best practice. Requirements Essential: Bachelor of Applied Science (Occupational Therapy) or equivalent recognised qualification. Registration with relevant professional bodies. Demonstrated ability to assess, implement, and evaluate therapeutic interventions. Strong communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team. Commitment to delivering high-quality care for older people and individuals living with disability. Current Driver Licence. Desirable: Experience in community-based OT, home care, or aged care settings. Knowledge of assistive technology, functional assessments, and home modifications. Experience in client education, carer liaison, or health promotion. Ability to contribute to service planning and development.
