As a Clinical Aged Care Assessor based in the Port Pirie region, you’ll conduct in-home assessments to identify each client’s unique needs, preferences, and goals. Helping others isn’t just a job, it’s who you are. Role Description Conduct comprehensive aged care assessments in clients’ homes, identifying individual needs and goals. Complete assessments and support plans within required timeframes, maintaining accuracy and quality. Connect clients with local service providers and community organisations that align with their needs. Collaborate with families and carers, recognising their vital role in the care journey. Ensure all assessments meet Department of Health standards and contractual obligations. What You’ll Bring At least 1 year working in aged care with hands‑on experience in client assessments and/or service delivery. Relevant qualifications: tertiary education in a health‑related field such as nursing, occupational therapy, physiotherapy or social work. Current unrestricted General Registration with AHPRA or an equivalent professional body. Comfortable navigating Word, Excel, Outlook and client management systems. Strong communicator who thrives on building trust and uncovering what older adults truly need to live well. People describe you as authentic, approachable, and endlessly caring.
