As part of the ForHealth Group, we’re focused on innovation, culture, and exceptional patient care.The RoleWe are looking for an experienced CDM (Chronic Disease Management) Nurse to join our Oxley Medical Centre to be the driving force and advocate for the local community in preventative health. This role will see you support GPs and their patients to identify and manage their chronic conditions and ultimately keep them out of hospital.The TasksIn this role, you will be involved in a wide variety of tasks including, but not limited to:Prepare Care Plans and Team Care Arrangements to help patients manage their disease/s Empower patients to increase ownership over their health Coach patients on embedding health behavior change principles Enable patients to access ongoing care from a multi-disciplinary team Assist in early identification of chronic disease by conducting health assessments Work with GPs to promote preventative healthcare throughout the practice Increase the number of patients who receive a care plan for the first time Ensure patient’s care plans are reviewed regularly Manage administrative tasks relating to carrying out CDM appointment There is an opportunity for bonuses The Successful CandidateAboutWe’re looking for nurses who are passionate about patient care and committed to working as part of a collaborative clinical team.
