Join to apply for the Care Partner role at HammondCare . We have a 6‑month position with full‑time hours, Monday‑Friday, for our Home Care team during our growth phase. What’s involved in the role Assist in providing services tailored to individual clients’ needs, delivered within budget and service expectations, including ongoing client assessment and the development of care plans in consultation with key stakeholders. Responsible for care management of existing clients and client‑related KPIs. Ensure care plans are up to date and clients receive appropriate care that corresponds with their needs, approval, and funding. Provide professional and competent day‑to‑day delivery of services in line with service expectations, and ensure staff understand each client’s needs and preferences. What do you need to apply Certificate IV in Aged Care and at least 2 years of relevant industry experience. Knowledge of the Home Care Packages Program, Support at Home, Commonwealth Home Support Programme and Aged Care Quality Standards. Experience operating effectively in a multi‑site environment. A current driver’s licence and willingness to travel between HammondCare’s clients, sites, and services (salary package may include a motor vehicle). Strong administrative and computer skills.
