Health Services Officer – Health Support Programs Role type: 12-month fixed-term contract (Wednesday and Friday). Reporting directly to the Clinical Services Manager, the Health Services Officer is responsible for managing the health-e-Profile, an online health risk assessment on behalf of TH, supporting the Health Monitor project and assisting TUH/UH member access to Chronic Disease Management programs. The Health Services Officer will also assist the Clinical Services Manager with population health reporting, progress reporting, and support TH to deliver on the strategic objectives and activities in the Chronic Disease Strategy. The Health Services Officer may be required to provide support to the Care Coordination Service during times of increased activity, planned or unexpected leave periods to ensure business continuity for the small team. Monitor the Health-e-Profile and Health Monitor dashboards and identify TUH/UH members with risk factors. Conduct outbound calls to eligible members to engage them in Chronic Disease Management Programs and complete referral processes. Develop and facilitate the delivery of automated health messaging. Respond to all relevant Health Support Program enquiries and provide pathways to information on support in a timely manner. Generate data reports to inform population health and chronic disease reports. Other duties as required to support the Clinical Services Manager to meet Health Care Services (Brisbane) operational needs. What do you need? Minimum of 2 years’ experience in an administrative role, preferably within the health services sector. Provide the highest level of customer service through accurate information and advice to members. Promote services and resources through targeted and effective communication to increase engagement in health initiatives. Proficient in Microsoft Word and Excel, and an ability to use a broad range of databases, information systems and business intelligence tools. Exceptional communication (written and verbal) and interpersonal skills. Demonstrated ability to analyse data and determine appropriate course of action. Experience within a data entry role with a high level of accuracy. Prioritisation of work, including the ability to manage multiple tasks. Ability to work autonomously, but also collaboratively as a member of a team. How do I apply?
