About The Role We are looking for Youth Lead Practitioners to support adolescents and young adults aged 10–24 as they navigate life with a disability, from adolescence through to early adulthood. Responsibilities Led the interdisciplinary team that plans a young person’s NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families Utilised assessment and intervention skills to support youth participants to achieve their educational, personal, social and working‑life goals Worked within the social model, delivering therapy in diverse settings from homes and schools to playgrounds, workplaces and community centres Held meaningful, future‑focused conversations with participants and their supports Empowered participants and their families to develop habits and routines Built meaningful relationships with participants and their supports (family, carers, friends) Documented and billed for each session through our online participant management system Attended the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs Met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or support Who We Are Looking For At Everyday Independence, our team thrives on shared values and a collaborative spirit. To work as a Youth Lead Practitioner, you will have a relevant tertiary qualification and registration.
