We’re Hiring: Clinical Nurse (Community Based) We’re looking for a dedicated and compassionate Registered Nurse to collaborate with our Home Care Package Coordinators, delivering high-quality clinical services to clients in their homes. In this role, you’ll support clients to live safely and comfortably while ensuring their clinical needs are met with professionalism and care. Lead clinical care planning, reporting, and clinical governance duties. Conduct thorough client assessments with a focus on client directed care. Coordinate and deliver person-centered care that aligns with client needs, care plans, and available funding. Provide in-home clinical services including clinical care plan assessments, medication support, catheter care, wound and complex wound management, chronic disease management, palliative care, PAS assessments, and diabetes support. Investigate clinical incidents and offer training or guidance to staff when required. Work collaboratively with the care coordination, allied health, and nursing team to ensure holistic client support. Who We’re Looking For Current AHPRA registration (Registered Nurse). Minimum 3 years’ post-registration experience in a community setting. Ability to work both autonomously and collaboratively. Current WA Driver’s Licence and reliable vehicle. NDIS Worker Screening Clearance. Current First Aid and CPR certification. Understanding of Aged Care and NDIS Quality Standards. Excellent written and verbal communication skills.
