Occupational Therapist — Transitional Aged Care Programme St Vincent’s Hospital, Darlinghurst, New South Wales, Australia. Maximum-term positions: Part‑time (32 hrs/week) until June 2027 and Full‑time until February 2026. Role grade: 1–2 Salary: $73,427.07 – $104,752.76 (Pro‑rata) + Superannuation About the Role Provide comprehensive Occupational Therapy assessment and evidence‑based treatment for clients on the Transitional Aged Care Program in the community. Identify occupational needs through standardised and functional assessments, designing tailored, patient‑centred interventions focused on therapy, discharge planning, and efficient patient flow. Manage cases effectively, liaising with community services, health‑care providers, and support networks to ensure coordinated care and appropriate referrals. Actively contribute to quality improvement and research initiatives within the Transitional Aged Care Program. Educate the Transitional Aged Care Team and other community groups, while responsibly managing occupational therapy equipment and organisational resources. About You Qualified and AHPRA‑registered Occupational Therapist with a full Class C driver’s licence. Proven experience in community settings and passion for delivering compassionate care to vulnerable populations. Confident in applying clinical reasoning and exercising sound professional judgment in a fast‑paced, multidisciplinary environment. Strong communicator who thrives in a team, building relationships with patients, families, and colleagues.
