Project Lead HealthPathways Dementia and Aged Care Murray Primary Health Network – Bendigo VIC Murray PHN is seeking an experienced General Practitioner to lead the HealthPathways Dementia and Aged Care Project. This role is pivotal in improving clinical referral pathways and supporting local health practitioners through consultation, education, and workforce development. There is flexibility to schedule your work week as you need. Key Accountabilities Lead consultation activities to gather insights that inform dementia and aged care clinical referral pathway improvements, identify system enablers and barriers, and shape workforce development initiatives. Facilitate and support workforce development education sessions for health practitioners on dementia and aged care pathways. Develop and deliver project reporting, including health system analysis and key themes, to demonstrate outcomes and inform future improvements. Educate and engage health practitioners on the use and benefits of dementia and aged care clinical referral pathways to enhance care quality. Collaborate with Clinical Editors to review and refine dementia and aged care pathways and implement quality assurance tools such as clinical audits. Monitor, report, and communicate progress to stakeholders, acting as a key representative for Murray PHN in promoting clinical referral pathways and addressing workforce needs. About You As a flexible 0.2 FTE role, this position is ideal for a GP who wants to contribute their expertise to improving dementia and aged care pathways while maintaining balance. You’ll enjoy connecting with local health practitioners, consulting on system improvements, and providing education that makes a real difference in patient care. Key Capabilities Tertiary qualifications and a current AHPRA registration as a General Practitioner and an experienced, practicing GP. Strong communication and stakeholder engagement skills to lead consultations with health professionals, facilitate education sessions, and build collaborative relationships across the health system. Experience in project management and health system analysis to manage timelines, coordinate activities, and monitor outcomes on high-impact projects that improve clinical referral pathways and workforce capability. Ability to interpret and present information clearly for diverse audiences, translate complex clinical and system insights into practical resources and reports that inform decision-making and support practitioner education.
