Support Time & Recovery Navigator Practice Plus Group is looking for a Support Time & Recovery Navigator to join our Reconnect team in South Yorkshire. At Practice Plus Group we put people’s wellbeing first. Our Health in Justice service line is expanding into non‑custodial healthcare, adopting a person‑centred approach delivered through a consolidated infrastructure aimed at assessing individuals’ mental health and neuro‑diversity requirements and redirecting them to the most appropriate channel. About The Role This is an opportunity you don’t want to miss. Key Responsibilities Part of an extended team working across Liaison & Diversion and Reconnect services, liaising with police, probation, courts, legal services, inpatient and community health and social care services. Provide daily support of children and adults, especially from hard‑to‑reach communities, to engage and navigate through the Criminal Justice System. Provide practical support to individuals, maintain a programme of support, work with their families, advocate and enable access to agencies including probation services, community‑based organisations, clinics, courts, mental health services and hospitals. Experience of working with service users face‑to‑face, with a variety of health needs. Understanding of the complexities of multi‑disciplinary and multi‑agency working. A non‑judgmental and compassionate approach. Salary: £29,263 to £33,510 per annum, depending on experience. Location: South Yorkshire – Doncaster.
