Purple House began 22 years ago with a knock-out exhibition at the Art Gallery of NSW that raised over a million dollars. From that moment, families in the most remote parts of the desert created their own organisation – one built on caring, compassion, and determination. In 2004, Purple House delivered its first dialysis service in Kintore – the first of its kind – and since then we have expanded across 20 remote communities in NT, SA, and WA. We also run two mobile dialysis trucks, provide a range of health and community support services, and have grown into a successful social enterprise with big plans for the future. Our mission is to improve the lives of people with renal failure, reunite families, and reduce the impact of chronic disease in our communities. We are run by Aboriginal people for Aboriginal people, guided by our values of Walytja (family), Tjukurrpa (dreaming), Ngurra (Country), and Kuunyi (compassion). About the Role The Chronic Disease Care Coordinator plays a key role in supporting patients with complex and chronic health conditions to access the right care, improve health outcomes, and strengthen connections between urban and remote healthcare services. Working as part of the Primary Health Care (PHC) team, you’ll provide case coordination and patient advocacy for individuals managing multiple chronic diseases — including those transitioning between remote and urban care or newly commencing dialysis. You will: Coordinate holistic, patient-centred care in line with PHC goals and community needs. Facilitate access to internal and external healthcare services, including GPs, specialists, and allied health. Support patients with appointment scheduling, transport, and ongoing care follow-up. Build strong communication with multidisciplinary providers to ensure seamless care delivery. Maintain accurate and timely patient records and documentation. Work closely with the Malpa team to provide a culturally safe and holistic approach to care. Promote and contribute to a safe, respectful, and quality-driven work environment. About You You are compassionate, flexible, and culturally aware, with a passion for improving health outcomes for Aboriginal communities. You are comfortable working both independently and as part of a close-knit team, and you take pride in providing high-quality, holistic care. To be successful, you’ll bring: Current AHPRA registration as a Registered or Enrolled Nurse (or equivalent). Experience supporting patients with chronic and complex care needs. Understanding of chronic disease management frameworks and health promotion. Strong communication, problem-solving, and interpersonal skills. Demonstrated cultural awareness and respect for Aboriginal communities. A current NT driver’s licence and National Criminal History Check (within 6 months). Basic Life Support (BLS) certification. Experience in Aboriginal community-controlled health or remote health settings will be highly regarded.
