The Role We’re searching for a confident and compliance‑minded Care Coordinator who thrives on structure, leadership, and behavioural‑support excellence. You’ll manage a caseload of complex participants and lead support teams to implement Positive Behaviour Support (PBS) strategies that truly work. You’ll collaborate with practitioners, families, and internal stakeholders to ensure every plan is followed with precision, empathy, and integrity. What You’ll Do Lead and mentor support teams implementing Behaviour Support Plans (BSPs) under NDIS Modules 2 and 2A. Review and analyse incident data, ensuring accuracy, transparency, and timely escalation. Support staff capability through coaching, supervision, and corrective training. Liaise with behaviour support practitioners, families, and external stakeholders to ensure continuity and effectiveness of BSPs. Drive quality improvement initiatives aligned with Concept Care’s compliance framework and the NDIS Commission standards. Maintain audit‑ready documentation, contribute to service reviews, and promote a culture of professional accountability. About You Experienced in behaviour support coordination or service delivery under NDIS Practice Standards. Skilled in interpreting Modules 2 and 2A, restrictive practices, and incident management. Confident leader who coaches others with calm authority and high emotional intelligence. Exceptional communication, analytical thinking, and time‑management skills.
