This position works in close collaboration with the nursing team to provide compassionate, person‑centred care through: Individual engagement and behavioural support tailored to each patient's needs Meaningful activities designed to ease agitation and promote emotional wellbeing Support for mobility, falls prevention, and sensory stimulation to enhance physical safety Preserving dignity and safety for patients experiencing dementia, delirium, or frailty Enhanced Care Activities Coordinators play a pivotal role in ensuring patients receive not only clinical attention but also vital emotional and psychosocial support—leading to fewer adverse events and a significantly improved patient experience. You will receive comprehensive training to confidently deliver Enhanced Care activities and provide meaningful support to patients.
